how to make complaining letter

i'm one of your loyal member in JED, i have got certificate of TMT and Ngaji tenses which was held on 1st August 2011. but i was surprising because my name incorrect. The officer spelled and wrote my name in a wrong way. it sholud be "LATHIFA" but in certificate is written as "LATIFA" without "h". i thought without "h" might give me difficulties in using certificate in the future. So, could you please make me a new certificate in my correct full name?? If you have finished to make it please contact me on my phone number 085xxxxxxxxx.
